Omega Louise Applegate's earthly journey ended on Nov. 25, 2009 at Banner Churchill Community Hospital after a long battle with Emphysema. She was born in Caddo, Cougar County, Okla. on Dec. 16, 1939 to George and Annie Madaris. She spent her life being a wife, mother, grandmother and great-grandmother. She had lived in Fallon for the last 10 years.
She was preceded in death by her parents and son, Carl Carlson.
She is survived by her husband, Wayne Applegate; daughters, Cheryl (Doug) Boren of Payson Utah, Wanda (Paul) Hiller of Eagle Mountain, Utah and Sue (Ken ) Adams of Fallon; son, Terry (Susan) Brown of Utah; grandchildren, Clay Boren, Floyd Boren, Bobbie Lee Boren, Carla Ann Boren, Becky (Allen) Curtis, Bradley Olsen, Jessica (Tim) Beeman, Earl Adams; and 19 great-grandchildren.
Arrangements are under the care of Smith Family Funeral Home and Crematory. No services will be held in Fallon. She will be buried on Dec. 5, 2009 in Santaquin, Utah.
